I’ve used my Kiev 60 again after quite a while, and unfortunately, after the development of the Fomapan 200, I’ve noticed some strange defects on the negatives.
Here’s an example – scanned using Vuescan, simply resized and not otherwise edited:

I’ve had relatively few problems with the Kiev so far (I’d got the issue with the shutter curtains under control) and at first glance it seems to be working fine, so I think something must have gone wrong during development. I developed it for 12 minutes using Caffenol-C-M-rs. I’ve already developed various Fomapan films in Caffenol, mostly with success so far.
I’ve been using the formula from https://caffenol.blogspot.com/ with very good results so far. I also develop using Adonal and C-41 chemicals from Tetenal.
I wouldn’t describe myself as completely clueless when it comes to negative development, but I’m no absolute expert either. If anything went wrong, I’ve usually been able to pinpoint the fault as being either with me or the camera. This time, however, I’m unsure.
The defects are visible across the entire roll; in some places, it looks as though the film has been exposed to light. I must admit that it sat on a shelf in my room for a few days, neatly rolled up but not wrapped in anything extra. If that’s what caused the defects, at least I’d know it was my fault again. ;)

...next time, just give a standard developer a go – something like D-76 or similar – and forget all about the Caffenol hype and the Rodinal 1+100 tank development hype while you’re at it – unless, of course, you’re into surprises...

It looks like a faulty development.
The development is very uneven; there are patches and streaks visible.
There is a severe lack of contrast, there is a degree of granularity, and it lacks sharpness.
This negative has several problems.
I would fully agree with Wolf’s recommendation.
You’d be better off trying a different developer.
The film used may also not be entirely problem-free.
That could possibly be a (small) part of the overall issue.
The camera can be ruled out as the cause of the problem.
