Taking a wild guess from a distance, I’d imagine that the film wasn’t bleached sufficiently.
In that case, too much metallic silver from the initial development would have remained in the emulsion. This would mean that the areas of the positive that should actually be light would remain dark.
I used to have such mishaps with my ‘conventional’ process (five to six baths, including a separate Bleach bath) when the Bleach bath (potassium permanganate) was no longer up to scratch.
